Refer a Patient

Step 1: Complete the Secure Referral Form

Please complete the HIPAA compliant referral form and provide as much relevant information as available.

Step 2: Patient Outreach

Once the referral is received, I will review the information and reach out to the patient within 1 business day to begin the scheduling process.

Step 3: Psychiatric Evaluation and Ongoing Care

When clinically appropriate, I provide psychiatric evaluation, diagnostic clarification, medication management, and ongoing follow up care through secure telehealth appointments.

I value collaboration with referring providers and work to support continuity of care while maintaining patient privacy and consent requirements.

Referrals May Be Appropriate For Patients Needing Support With:

• Depression

• Anxiety

• ADHD evaluation and treatment

• Mood disorders

• Trauma related symptoms

• Sleep concerns related to psychiatric conditions

• Medication evaluation or medication management

• Diagnostic clarification

• Ongoing psychiatric follow up

If a patient is experiencing a psychiatric emergency, active suicidal intent, homicidal intent, psychosis with safety concerns, severe substance withdrawal, or requires immediate stabilization, please direct them to the nearest emergency department or call 911.
